Overview
A collision report contains information about a specific collision, including:
- names of the drivers involved
- summary of the road conditions at the time of the collision
- make and model of the vehicles involved
- description of the collision
Collision reports are usually needed for insurance or legal reasons.
Before you order
To order a collision report you will need the:
- date of the collision
- driver鈥檚 licence number or collision report number
- enter details if available, for example plate number(s) and collision location
Cost
You can choose to order:
- an uncertified report for $12 (most common)
- a certified report for $18
An uncertified report will meet most needs.
A certified report includes a seal from the Ministry of Transportation and is typically required for legal reasons only.
Order a report
You can order a collision report online or by mail.
If the collision happened before July 1, 2014, the report will be mailed to you in 6 to 8 weeks.
Online
The report is available to download immediately if the collision happened:
- more than 30 days ago
- on or after July 1, 2014
It takes 20 to 30 calendar days to upload reports from recent collisions into the online system. Please wait 4 to 6 weeks before you order online. If a collision report is not found, no fee will be charged.
Be ready to:
- enter the date of the collision
- enter the driver鈥檚 licence number or collision report number
- enter details if available, for example plate number(s) and collision location
- pay with a valid Visa or Mastercard credit card
- print or save the report immediately

When you order by mail, you can pay with:
- Visa or Mastercard credit cards
- personal cheque or money order in CAD funds payable to the Minister of Finance (by mail only)
Your order for an uncertified or certified report will be processed within 15 business days and then sent by regular mail via Canada Post, if the collision happened:
- more than 30 days ago
- on or after July 1, 2014
Your privacy
To protect a driver鈥檚 privacy, collision reports do not include a driver鈥檚 residential address. Anyone can order a collision report if they provide the required information.
